Bismuth Subsalicylate

Indications

Bismuth Subsalicylate is indicated for diarrhea, nausea, heartburn, indigestion, and upset stomach.

Description

Bismuth subsalicylate is used as an antacid and antidiarrheal, and to treat some other gastro-intestinal symptoms, such as nausea. The means by which this occurs is still not well documented. It is thought to be some combination of the following:

• Retarding the expulsion of fluids into the digestive system by irritated tissues, by "coating" them.
• Stimulation of absorption of fluids and electrolytes by the intestinal wall (antisecretory action).
• As a Salicylate, reducing inflammation/irritation of stomach and intestinal lining through inhibition of prostaglandin G/H Synthase 1/2.
• Reduction in hypermotility of the stomach.
• Binding of toxins produced by E. coli.
• Bactericidal action of a number of its subcomponents, including salicylic acid.
• Bactericidal action via a so-called oligodynamic effect in which small amounts of heavy metals such as bismuth are toxic for a number of microbes.
• Weak antacid properties.

Pharmacology

The demulcent base provides a protective coating of the lower oesophagus and a partial coating in the stomach which holds the Bismuth Subsalicylate in suspension. Bismuth subsalicylate may exert its antidiarrheal action not only by stimulating absorption of fluid and electrolytes across the intestinal wall but also when hydrolysed to salicylic acid, by inhibiting synthesis of a prostaglandin responsible for intestinal inflammation and hypermotility. In addition, Bismuth Subsalicylates binds toxins produced by E. coli.

Dosage & Administration

Adult: 30 ml 3-4 times daily 30 minutes before meal.
Child: 3-12 years: 5-15 ml.

Interaction

There are no known drug interactions and none well documented.

Contraindications

Hypersensitivity to bismuth, aspirin, other salicylates, Infectious diarrhea, high fever, von Willebrand disease, hemorrhage, ulcer or Gl bleeding with black or bloody stool, hemophilia, chicken pox or influenza (risk of Reye syndrome); changes in behavior with nausea and vomiting may be early sign of Reye syndrome.

Side Effects

Anxiety, any loss of hearing, confusion, constipation (severe), diarrhea (severe or continuing), difficulty in speaking or slurred speech, dizziness or lightheadedness drowsiness (severe), fast or deep breathing, headache (severe or continuing), increased sweating, increased thirst mental depression, muscle spasms (especially of face, neck, and back), muscle weakness, nausea or vomiting (severe or continuing), ringing or buzzing in ears (continuing) stomach pain (severe or continuing), trembling, uncontrollable flapping movements of the hands (especially in elderly, patients) or other uncontrolled body movements vision problems

Pregnancy & Lactation

Pregnancy Category-C. Animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in humans, but potential benefits may warrant use of the drug in pregnant women despite potential risks
